How do I submit a referral?

What is Sequoia?

I forgot my Sequoia username/password. What do I do?

To retrieve your login information, or to have your password reset contact your account manager, email support@cypresscare.com or call 800.419.7191

I don’t have a Seqoia login. How can I get one?

Do you offer CEU presentations?

Yes. Cypress Care offers a variety of CEU programs. In addition to in-office CEU presentations we also offer quarterly CEU Webinars. Click here (link) to join the mailing list and be included in future webinars. If you have idea suggestion for a webinar topic, let us know!

How do I submit a pharmacy referral?

You can submit a referral online, via email, by telephone at 800.419.7191 or via fax at 800.419.7194.

Does a claimant have any out of pocket expenses while using Cypress Care?

No. The Cypress Care pharmacy program is designed to eliminate all out pocket expenses for claimants.

What do I do if a claimant loses the Cypress Care pharmacy card?

Contact customer support at 800.419.7191 or via email at support@cypresscare.com and a new card will be mailed to the claimant on the next business day!

Do you have a first fill program?

Yes. Cypress Care offers a first fill program that immediately provides essential prescriptions to injured workers while claim eligibility is determined. This program can be customized to fit your business needs.

Do you have a mail order program?

Yes. Cypress Care’s mail order program, EzRx, is designed to offer claimants up to a 90 day supply of medications with no out of pocket expense. You can sign a claimant up for mail order online, via email or by calling 800.419.7191.
