Clinical Care Coordinator
Purpose/Objective:
The Clinical Care Coordinator will support corporate policies, programs and practice thorough communications and application in work environment. The Clinical Care Coordinator’s role requires a high degree of integrity, confidentiality, as well as strong interpersonal skills, organization, professionalism, and initiative.
Responsibilities and Functions
Directly reports to the Clinical Ancillary Services Supervisor and Clinical Ancillary Services Operations Manager. The Clinical Care Coordinator is responsible for the following:
- Complete Intake Orders for Clinical Services (primarily home health services) Input patients into system and create orders
- Answer questions on orders (owned by yourself or others) from adjusters or Nurse Case Managers
- Locate vendors within network or in patient’s area to provide clinical services
- Coordinate discharges
- Manage Home Health Cases (notes, liaison for communication with agency and Nurse Case Manager, continued auth, etc.)
- Negotiate pricing and services with vendors
- Manage email inquiries from sales personnel
Qualifications
- Must be a licensed Registered or Practical/Vocational nurse with current Georgia nursing license
- 5-7 yrs. Case Management, Discharge Planning, or Home Health experience preferred
- Workers Compensation insurance experience a plus
Skills/Ability To
- Ability to work independently and as a team member in an office setting
- Self motivation required
- Must be familiar with Microsoft based programs
- Strong customer service skills required
- Ability to adapt work flow within a dynamic work environment
- Ability to take direction and work in collaborative relationships with internal and external customers
- Strong verbal and written communication skills
- Strong attention to detail required
- Must have excellent phone skills
